How to Book and pay for your Assessment

To book an assessment please contact our appointments team on 01787 222 873, alternatively please complete the referral form and return to us, in order to receive an application form.

Application form and payment must be received prior to an assessment being booked. Payment can be made by cheque, BACs or over the phone using a debit or credit card.
